Northbank Surgical Center is now offering online pre-registration for your surgery.  Registration is designed to be quick and easy and completing the form should take you 10-15 minutes.  Please have your insurance information handy.

Thank you for choosing to have your surgery with us at Northbank Surgical Center.  It is very important that we contact you prior to your procedure.  We will start to contact you within 4 to  5 days of your surgery and continue up until the business day before your surgery.  Please call our office at 503-364-3704 if we do not reach you by 2pm the business day prior to your surgery date. 

Your surgeon’s office will give you an admission packet that includes pre-registration forms should you choose not to register online.  Please complete the forms and brings them with you on the date of your surgery.

Northbank Surgical Center’s policy on Advanced Directives.  As your procedure here is elective, the Surgery Cener does not honor advanced directives.  In the event of an emergency that necessitates your transfer to a hospital we would either forward the copy you bring with you or provide the information to your next care giver.
